Abstract
The purpose of this study was to identify the effect of hospital based home care activity. Study subjects were 170 patients received home care service from 1st May, 1997 to 31st, Dece., 1998. The data were collected by anecdotic record and analyzed using frequency, percentage, mean, and t-test by SPSS PC + program.
The results were as follows.
The ages of subjects were mostly sixties and more. According to the classification of disease, cancer patients were 60% and patients of cerebral vascular disorder were 13.5%. Most patients registered to receive hospice care and keep their condition through palliative care. The persons who referred them to the home care service were physicians in changing of patients treatment, patients themselves and their family. Mostly health problems when patients registered were classified as level ¥±. Total numbers of home visits were 1718. The mean number of home visit for all subjects was 10.11. The mean number of home visit for cancer patient was 11.03 and for patient with cerebral vascular disorder was 8.7. The mean duration of visit for one subject was 46.89 days. The mean time of giving care was 45.69 minutes per one visit. The mean time of taking one-way transportation was 26.85 minutes. The mean time of taking home visit for one subject was 99.39 minutes. The subjects who terminated the visit because of death were 60%. Heath costs and hospital days of home care cancer patients were less than of hospitalized cancer patients. According to the content analysis of home care service there were 108 types of nursing interventions. The most frequently done services among them were like assessing vital signs, fluid therapy, taking health history, assessing pain and diet instruction.
These finding suggested that the patients had substantial need for home care because they tend to be an elderly and have experienced the wide range of health problem associated with chronicity. In order to increase quality of home care, it is necessary to input specialist and give continued education for home care nurse. It is placed an emphases on hospice care and palliative care to home care nurse. It is concluded that home care is developed having effect of hospitalization and also it is needed to reestablish role of home care nurse,
